Healthy eyes and clear vision play a vital role in your overall health and quality of life. Since most health insurance plans don’t include vision care, enrolling in a separate vision plan can help you support and maintain your overall physical well-being.

Plan Features

Most vision plans include coverage for one eye exam per calendar year and offer additional benefits to help reduce the cost of lenses, frames, or contact lenses. INDEMNITY PLAN

You are free to visit any provider you prefer; however, you will be responsible for paying the full cost of care at the time of service.

VISION PPO

Visiting an in-network provider can help you save money, as these providers have agreed to discounted rates. This means you'll pay less out of pocket when you receive care.

VISION HMO

You’re generally required to choose an in-network primary care provider for vision services and receive care exclusively through them. Coverage for out-of-network providers is not available.
